Plot Summary

Emily Jenkins (Zellweger) is a social worker who is assigned to the case of Lilith Sullivan (Ferland), a 10-year-old girl who lives with her parents Edward (Callum Keith Rennie) and Margaret (Kerry O'Malley). Emily suspects that Lilith is being abused by her parents, who claim that she is evil and has supernatural powers. Emily visits Lilith's home and finds her locked in the oven, while her parents try to burn her alive. Emily rescues Lilith and arrests her parents, who are sent to a mental institution.

Emily decides to take care of Lilith until she finds a suitable foster family. She develops a bond with the girl, who seems sweet and innocent. However, strange and terrifying things start to happen around Emily and those close to her. Her friend and colleague Doug (Bradley Cooper), a child psychologist who interviewed Lilith, is attacked by a swarm of hornets that emerge from his mouth, causing him to crash his car and die. Emily's boss Wayne (Adrian Lester) is stabbed to death by an unknown assailant in his office. Emily's other friend and colleague Barbara (Cynthia Stevenson) is killed by her own dog, who mauls her to death.

Emily begins to suspect that Lilith is behind these deaths, and that she is not a normal child, but a demon who feeds on fear and misery. She contacts Detective Mike Barron (Ian McShane), who investigated Lilith's case, and asks for his help. Mike agrees to help Emily, but he is also killed by Lilith, who sets his house on fire. Emily realizes that she has to kill Lilith in order to stop her evil deeds.

Emily confronts Lilith in her home and tries to drown her in the bathtub. However, Lilith fights back and reveals her true demonic form. She tells Emily that she chose her as her "mother" because she was lonely and unhappy. She also says that she can make Emily's deepest wish come true: to have a child of her own. Emily rejects Lilith's offer and manages to subdue her with a syringe of sedative. She then puts Lilith in a suitcase and drives to the hospital where Lilith's parents are held.

Emily meets with Edward and Margaret, who beg her to kill Lilith. They explain that Lilith was not their biological daughter, but an orphan they adopted from an agency that specialized in finding homes for troubled children. They say that they tried to love Lilith, but she turned out to be a monster who tormented them and killed their other children. They also say that they tried to kill Lilith several times, but she always survived.

Emily gives them the suitcase with Lilith inside and tells them to do what they have to do. She then leaves the hospital and drives away. However, she soon hears Lilith's voice in her car radio, taunting her and saying that she will never get rid of her. Emily looks at the back seat and sees the suitcase open and empty. She screams in horror as Lilith appears behind her and grabs her neck.
